PARK FAMILY EYE CARE

EyeMed Vision Plans is one of the fastest growing vision plans. EyeMed has a large number of programs that provide either discounted fees or reimbursements for routine, non-medical vision care and materials. Some of the EyeMed plans simply provide a percentage discount off the usual and customary fees at the practice. Other EyeMed plans have more elaborate and extensive coverage that provides allowances and/or discounts on glasses, and/or contact lenses, and vision care services. It is critical that you inform our office staff at the time you schedule your appointment that you have EyeMed. This enables our staff to contact EyeMed in advance of your scheduled visit to obtain your specific benefits description. Depending on the specifics of your EyeMed plan, contact lens professional services may not be a covered expense. A contact lens diagnostic evaluation is not considered part of a routine comprehensive eye examination by EyeMed . The fee for the contact lens diagnostic evaluation will vary according to the complexity of the case. Some EyeMed plans will cover both a comprehensive eye examination and a contact lens evaluation. Other EyeMed plans do not cover the diagnostic evaluation, and it must be paid for by the member. If a new material or design of contact lens is fit, there is a need for professional contact lens after care or follow up to make sure that the lenses are functioning properly in terms of vision, comfort and eye health response. The fees for contact lens after care also vary according to the complexity of the specific case involved. After care or follow up care is typically not paid for by EyeMed and is also an out of pocket expense on the part of the member. Most EyeMed plans provide some sort of benefit for contact lens materials. This would be in the form of a dollar "allowance," which varies from plan to plan, and/or a percentage discount from the usual and customary contact lens material fees.

Most EyeMed plans provide coverage for either contact lenses or glasses NOT BOTH. In cases where you may desire to purchase both conact lenses and glasses, PFEC will extend a 20% discount on glasses if you have used your EyeMed contact lens allowance for your contact lens supply.

Medical eye care services are not covered by EyeMed. If you require medical eye care services, we will work along with your medical insurance carrier to help you receive your maximum benefit. Examples of conditions covered under medical coverage include: diagnosis and treatment of eye infections, injuries, glaucoma, retinal problems, etc. As an added service to you, PFEC through its billing and coding service, will submit all medically related eye service claims directly to your medical insurance carrier at no charge. If we are preferred providers in your medical plan the we will accept payment from your carrier. If we are not, we will still submit the claim for you after you pay for your services directly to our office. Insurance payment will then be made directly from your carrier.

No matter what your specific EyeMed contact lens benefit, you can be assured that PFEC, the preeminent contact lens practice in the western suburbs of Chicago, will provide you with the highest quality of care as it relates to contact lens services and materials.
